Facilitation?

Resource Facilitation is a free service designed to help individuals with brain injuries—and their families—navigate life after injury by connecting them to the supports they need.

Whether someone is newly injured or adjusting to long-term challenges, a Resource Facilitator works one-on-one to:

  • Identify needs and goals
  • Connect individuals to local services like housing, employment, therapy, benefits, and more
  • Support communication between medical providers, caregivers, and agencies
  • Promote independence and quality of life
  • Advocate for individuals to receive appropriate care and services

Who Is It For?

Resource Facilitation is for anyone in Utah who has experienced a brain injury—including concussions, strokes, and traumatic brain injuries—as well as their caregivers.


How It Works

You’ll be matched with a trained facilitator who will work with you step-by-step to:
  1. Understand your goals
  2. Explore resources in your area
  3. Build a plan to move forward
  4. Stick with you along the way
